With your Advance Directive (Power of Attorney for Health Care) you appoint someone to make medical decisions for you if you cannot make them yourself. Without it, a probate judge will appoint someone to make decisions for you. Often, your Advance directive will, also, indicate your wishes regarding end of life treatment, including when it should be discontinued.
