An Appointment of Funeral Representative authorizes the person that you have named to make decisions regarding your funeral arrangements and the handling, disposition, or disinterment of your body. This includes, but is not limited to, decisions about cremation, and the right to retrieve from the funeral establishment and possess cremated remains immediately after cremation. We include a place for you to indicate your preferences for funeral arrangements, the handling, disposition or disinterment of your body, cremation, or possession of your cremated remains.

  1. Send us information about you and your Funeral Representative(s) by clicking the “Submit Information Here” button. You will need your Funeral Representative(s) name, complete address, and telephone number.
  2. Send us a copy of your ID (i.e. driver’s license) through our Documents Upload Center.
  3. Pay $215* retainer fee HERE.

*Fee will be less if you are a member of an organization where our services are provided as a member benefit.
